Mission Statement
The Our Lady of the Lake University Athletic Training Department exists to provide health care services to all student-athletes who participate in OLLU Athletics. The OLLU Athletic Training Department strives to prevent injuries, to provide prompt treatment of injuries when they occur, and to rehabilitate athletes whose injuries require restorative care. It will be the goal of the Athletic Training Department to administer

its responsibilities with both professional skills, compassionate care, and provide the best possible medical resources necessary to allow our student-athletes to return to competition as soon and as safe as possible.
 

Physical Information

All athletes are required to get a pre-participation physical completed and fill out the athletic training room required forms before they are eligible to participate in intercollegiate athletics. The required forms can now be obtained online and then printed out for completion and mailing.
  • Any person who wishes to participate in Our Lady of the Lake University Athletics, whether having been recruiting or not, is required to have completed the OLLU medical forms and pre-participation physical examination prior to being eligible to participate.
     
  • As an Our Lady of the Lake University athlete, you are required to complete and submit a medical history questionnaire, insurance information, waiver, and consent and release forms each year. Completion of all paperwork and the physical exam are required before you begin any athletic participation.
     
  • If you require a medical clearance for a health condition (i.e. heart condition) or injury (i.e. recent reconstructed ACL) from another physician, please arrange to obtain a signed release and to send in with you required forms below. If you have questions about if you should obtain a release, please contact the athletic training staff.
     
Insurance Information -- REQUIRED FORMS
It is the OLLU Athletic Department's desire that all our athletes have a safe and enjoyable experience while participating in any of our Athletic Training Room athletic programs. The highest priority for the Athletic Training Staff is to prevent injuries; however, there is always a risk of injury in any sport. In the event that the athlete is injured while participating in an NAIA Intercollegiate event, it is important for us to know about any medical coverage you may have in order to avoid delays in injury management and in claims processing. Therefore, as a requirement for any level of participation you just complete the "Athletic Insurance Information Form." 

 

Please note that it is important that all requested information be completed in its entirety. Also, any changes to your coverage must be reported and updated immediately to avoid injury o r claims management problems.

 
Our Lady of the Lake University does offer an Accident and Sickness Insurance Policy if you do not have personal insurance. This insurance policy is administered by Student Assurance Services (http://www.sas-mn.com/) and is underwritten by Columbian Life Insurance Company. If you choose to buy our school insurance please make sure you purchase the "Student-Athlete" coverage which will cost $650/year.
 

Please visit http://www.sas-mn.com/ to purchase the insurance. You can also download the brochure (listed below) for all the specifics. Mail in your check to the location listed on the enrollment form.


**If you are unable to purchase the insurance, please download and fill out the Request Form (listed below). With this form, you can purchase the insurance through your student account. Also, download and fill out the Student Assurance Services Enrollment Form (listed below), and send it in with your request form. When you have filled it out, please return it with all your completed paperwork. DO NOT turn it into the Billing Office. PLEASE NOTE: You can either enroll for the entire year at $650 or you can enroll at each semester. The costs are split up on the enrollment form.
