st sports medicine care. Head Athletic Trainer Chassity Muņiz is a state-licensed athletic trainer. She is responsible for the prevention, care and rehabilitation of injuries for the OLLU Saints athletic teams and for daily operations.
INSURANCE POLICIES and PROCEDURES:
It is important that every student-athlete is aware of the insurance policies and procedures at Our Lady of the Lake University.
The National Association of Intercollegiate Athletics (NAIA) requires all student-athletes to be covered by a personal or University sponsored insurance policy.
The information requested will assist the University in ensuring that each student-athlete is covered by medical insurance; especially as it pertains to sports-related injuries.
The University requires all students to maintain and show proof of medical health insurance for the academic year. It is the responsibility of each student-athlete to have in force personal medical health insurance or to enroll in the student insurance plan offered by the University. The University will not cover any student-athlete who does not maintain a primary health care plan.
Our Lady of the Lake University offers a Student Health Insurance Plan, which is available to students at a reasonable rate per semester. If you have elected or will elect the OLLU Student Health Insurance Plan as your primary insurance, please complete sections A, C, and E. If you declined or will decline the OLLU Student Health Insurance Plan coverage, please complete sections A, B, D, and E. There are parent/guardian signatures required on this page. For those declining the OLLU Student Health Insurance Plan, proof of insurance-a front and back photocopy of your insurance card- is required.
Unfortunately, not all insurance policies cover athletic injuries occurring in competitive sports. Some may have limited to no out-of-state or out-of-network coverage. Please check with your insurance company and indicate the information on the form as it pertains to your policy. All student-athletes that are covered under the TriCare Military or Medicaid insurance plan will be required to be on the student insurance plan through Our Lady of the Lake University. There may be some other insurance companies that student-athletes have that also may be required to purchase the insurance as well.
Additionally, section D lists Health Care Providers used by Our Lady of the Lake University. Confirm with your insurance carrier that these providers are or will be considered in-network in the event their services are required. As mentioned above, Our Lady of the Lake University does offer school insurance. Our insurance is called Pinnacle Student Insurance which is administered by Student Assurance Services Incorporated. Here you will find our student insurance enrollment form as well as the student insurance waiver form. If you will be purchasing the student insurance, please fill out the form and mail it to Student Assurance Service. If you are NOT purchasing the insurance please fill out the waiver form and return it to the Athletic Trainer as soon as possible.
Our Lady of the Lake University provides a secondary insurance plan for its student-athletes. THIS POLICY IS SECONDARY TO, OR IN EXCESS OF PERSONAL FAMILY MEDICAL INSURANCE COVERAGE. For the secondary coverage to be accessed, the student/athlete must have stayed within the guidelines of their primary carrier. The secondary insurance covers only injuries/illness/accidents resulting from the direct participation in the intercollegiate athletic program during the dates of the primary competitive season and designated off-season programs as approved by the Athletic Department according to NAIA guidelines.
The Athletic Training Department should be made aware of any pre-existing injuries, as the secondary insurance or Our Lady of the Lake University will not cover these conditions.
Please also note that the NAIA provides a Catastrophic Injury Insurance Program to cover student-athletes who are catastrophically injured while participating in a covered intercollegiate athletic activity (subject to all policy terms and conditions).
Finally, if there are any changes in your insurance coverage, please notify the Athletic Training Department immediately to avoid or lessen delays in claim processing. By not having the proper insurance information on file could also cause the claim not able to be filed at all. You are your best advocate in the claim payment process. Please make sure that medical bills you receive are brought into the Athletic Training Department without delay. Do not hesitate to inform us of any difficulties you are experiencing in getting a claim processed.
REQUIREMENTS and INSURANCE FORMS
The House Bill (HB 4189) recently passed requires that all incoming freshmen, transfer and international students living on campus must have the Bacterial Meningitis Vaccination. Deadline to get the shot is January 2010, but all student athletes are encouraged to get the shot before that date as the price will go up. After the deadline, students will need to have proof they have received the shot in order to live on campus.
An up-to-date Immunization Record for each student athlete is required.
