This information is utilized by the sports medicine department's athletic trainers and team physicians for your medical clearance and best possible coordination of your medical care while participating at OLLU. To avoid unnecessary delays in your clearance and sport participation, please turn in your forms before thestated deadline. If you have any questions concerning these forms or your clearance, please feel free to contact the athletic training office. It is very important that the forms are accurate and complete.
